Feel Your Best for the Upcoming Holiday Season!

Beginning November 5th, “Living Well with Chronic Conditions” will be offered FREE as a 6 session series. This program will offer tips and skills to help individuals manage problems common to chronic health conditions as well as skills to work effectively with health care professionals. The program also helps individuals take day-to-day responsibility for the care of their chronic condition. The information presented in the program is research-based and was developed by Stanford University. Anyone living with a chronic condition or family member, friend, or caregiver of an individual with a chronic condition can participate in this program. Living Well with Chronic Conditions helps people with common chronic conditions such as: asthma, COPD, arthritis, depression, diabetes, cancer, heart disease, obesity, and fibromyalgia. The program is being offered each Monday, November 5 to December 10, from 10:00 a.m. – 12:00 p.m. at the Jefferson County UT Extension Office. Registration is required as class size is limited.
